2008/09 Reservation Request Form

•	Education performances are exclusively for student groups.
•	One complimentary ticket is provided for every 15 tickets purchased.
•	Scholarships are available, click here for more information.
•	Within ten business days of placing a reservation, a 25% nonrefundable deposit (on reservations exceeding $100) and signed invoice are due. Reservations made before September 1, 2008 must return a signed invoice within 10 business days but have until September 19 to pay the deposit.
•	Final ticket counts and payments are due no later than three weeks prior to the performance.
•	Refunds or exchanges will only be honored if an event is cancelled by the Fox Cities Performing Arts Center or if your school is closed or delayed on the day of the performance.
•	Use the suggested grade levels listed for each performance as a guide to determine what is appropriate for your students. We recommend that children be at least three years old. Babies in arms will not be permitted in the theater.
•	Due to the nature of theatrical bookings: prices, shows, artists, dates and times are subject to change.
