Scholarship Request Policy
Please read the guidelines below before completing and submitting the Jan Bax Family Foundation Scholarship Fund Request Form.
Financial assistance is available through the Jan Bax Family Foundation Scholarship Fund for schools, educational and not-for-profit organizations in northeastern Wisconsin wishing to attend a Thrivent Financial for Lutherans Foundation education series performance. Please note that some performances may be excluded due to availability or other factors. Distribution of scholarship funds is managed by the Fox Cities Performing Arts Center’s Community Support Committee which evaluates the financial need and objectives of the request. Funds are awarded on a first-come, first-serve basis and are subject to availability.
Requests must be made a minimum of six weeks prior to the requested performance date.
All requests must be made utilizing the Jan Bax Family Foundation Scholarship Fund Request Form. The application may be submitted electronically or printed to be mailed or faxed to the Fox Cities Performing Arts Center’s Community Support Committee. Incomplete applications will not be considered.
Applicants must be a school or legally incorporated as a not-for-profit organization with IRS 501 (c) 3 status.
